$98,010.00 - $137,218.00The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
Responsibilities:
The School Based Health Center (SBHC) Family Nurse Practitioner (FNP)/Pediatric Nurse Practitioner (PNP) provides comprehensive adolescent health services to enrolled students, ages 10-21, in the school-based health center. The NP works in collaboration with the SBHC NP Supervisor/Medical Director. The Nurse Practitioner (NP) maintains a patient centered, customer-oriented approach to meeting patient needs and involving the family in the plan of care as appropriate. He/she maintains confidentiality and patient privacy. The NP works collaboratively with other SBHC providers and ancillary staff as well as community Primary Care Physician’s (PCP’s) and school personnel. The NP provides mental health assessments and medication management as clinically indicated. This is a 10-month position, September 1st to June 30th.ESSENTIAL FUNCTIONS
- Provides direct care through assessments, diagnosis, treatment planning, interventions and medication management, and management of acute illnesses and injuries, management of chronic conditions, health screenings and education, population-based primary prevention and monitoring outcomes of care.
- Provides adolescent health services and counseling that include but not limited STI testing and treatment, emergency contraception (Plan B), Birth control, Pregnancy testing and counseling, other sexual health services.
- Obtains and/or reviews/evaluates a patient's medical and psychosocial history and through patient evaluation and diagnostic studies, is able to determine a differential diagnosis and plan for intervention.
- Provides medical services to patients in accordance with UR and SBHC clinical policies and procedures and documents care provided, meeting UR, DOH and Medicaid billing requirements.
- Collaborates with appropriate members of the health team to facilitate the highest quality of services. Makes referrals to appropriate sources/consultants for medical and/or social problems.

MINIMUM EDUCATION & EXPERIENCE
- Master’s Degree from an Accredited Nurse Practitioner Program
- More than 3600 hours of qualifying nurse practitioner practice experience. Required
- Experience working with adolescent population (clinical rotation &/or work), interest in adolescent mental health, comfortable working with a small team, collaborative, team player. Preferred.
KNOWLEDGE, SKILLS AND ABILITIES
Clinical Competence from very acute, chronic and well patients Required
Fluency in Spanish language. Preferred.
