QUALIFICATIONS/SPECIFICATIONS/EDUCATION: Graduate of an approved school of practical nursing with current New York State Practical Nursing license. Possess good organizational, leadership and communication skills. By law is responsible for their acts and those acts they delegate to others.
ESSENTIAL FUNCTIONS:
- Ensures the implementation of resident care plans.
- Supervises CNA’s to ensure that resident’s care plans are followed.
- Transcribes and carries out physician's orders and integrates them into the resident's care plan.
- Assists in providing direct personal care to residents as needed.
- Helps maintain residents at their highest level of functioning by using rehabilitative nursing
principles.
- Assists with therapeutic and diagnostic procedures.
- Gives medicines and treatments. Restocks medication and treatment carts at the end of each
tour of duty.
- Makes out CNA assignments and gives CNAs verbal report.
- Communicates resident's condition by notifying nurse manager or supervisor. Communicates changes in residents’ conditions to all staff and families, as condition warrants. Maintains
pert sheet; records in resident's medical records and gives report to on-coming shift.
- Assists with admissions and discharges.
- Assists with MD day by making rounds with MD and transcribing orders.
- May accept telephone orders by physicians if LPN has been oriented and knows JHR policy.
- Supports facility and departmental policies.
- Maintains residents' confidentiality.
- Acts as the charge nurse who coordinates the daily operations of the unit when requested.
- Maintains a safe environment, conducive to positive health care and comfort, for
residents and staff in accordance with established policies and procedures.
- Assists in data collection related to residents care as directed by individual care plans and physician orders and documents per protocol.
OTHER FUNCTIONS:
- Keeps informed of new developments in their profession through reading, attendance at
In-service programs and staff meetings.
- Assists with unit quality assurance activities.
- All other job duties as assigned.

EXPERIENCE: Six (6) months to one (1) year experience as a staff nurse preferred. Long-
term care experience preferred.
RELATION TO OTHER POSITIONS: Daily contact with and ability to interact courteously and tactfully with. Staff, residents, family members, volunteers, visitors, physicians and contact with outside organizations.
PHYSICAL AND MENTAL DEMANDS: Sits rarely. May be required to lifts (and/or move) 50-100 pounds. Frequent twisting, bending, squatting, reaching, walking, pushing and pulling large objects, kneeling. All physical demands involved in direct nursing care. Subject to frequent interruptions. Must be able to cope with the demands of the position. Must function independently and be flexible. Physical work intensity level 4.
WORK ENVIRONMENT: Works in well-lighted, ventilated areas as well as throughout the nursing service areas, i.e., nurses' station, resident rooms, medication rooms, etc.
ADVERSE WORKING CONDITIONS: May be exposed to infections, diseases and disagreeable odors. May be subject to hostile and emotionally upset residents and family members.
VARIABLES: Required to work weekend and holiday. May be asked to work beyond normal working hours. May be asked to work on a different shift or unit than assigned.
