About the Role:
The Connections to Care Coordinator will focus on reaching individuals in our community experiencing mental health or substance use issues by interacting in community health fairs, visiting homeless shelters, presenting at local community centers, participating in consortiums around homelessness/housing instability, and collaborating with local community health centers. These opportunities for engagement will allow the specialist to share details about available services and help individuals connect with treatment. The Connections to Care Coordinator will assist individuals in connecting with services by attending appointments with clients as needed. The Connections to Care Coordinator will continually assess ongoing treatment needs.
Key Responsibilities:
- Participates in organized outreach events and connects with clients in the community
- Presents information about available programs at CCFCS and other agencies in the community
- Completes basis substance use screenings
- Links clients with needed services and accompanies clients to initial appointments as needed
- Actively seeks information to understand service provider/customer/staff needs and expectations.
- Builds rapport and fosters cooperative relationships with clients by establishing mutual respect
- Maintains open communication with staff around barriers or challenges
- Communicates and partners with the department and agency personnel as required to ensure on-going and effective program operations
- Establishes and promotes trust to facilitate collaboration - shares information with others
- Fulfills grant requirements
- Submit client encounter surveys to OASAS in timely manner
- Enter contact notes into electronic medical record as needed
- Actively participate in meetings with OASAS when scheduled
- Takes the initiative and leverages available resources (individuals, processes, departments, and tools) to complete their work efficiently and effectively
- Acknowledges mistakes right away and acts quickly to identify and implement problem resolution
- Ensures most critical tasks are always completed on time. If there are obstacles to completing them, does all they can to creatively problem solve. Informs management ahead of time if efforts are not enough to keep the tasks on track
- Maintains composure under stressful, distracting, or challenging situations
Qualifications:
- Bachelor’s Degree in Social Work or related field preferred.
- Two to five years of experience providing direct services in a human service organization.
A relevant combination of education and experience will be considered.
Preferred Skills:
- Must be proficient with Microsoft Office Suite.
- Experience working in multicultural environments and/or human services field is preferred.
- Ability to speak Spanish is preferred.
