Job Title: Licensed Practical Nurse (LPN) – Outpatient Cardiology
Location: Brighton – Clinton Woods (Primary), with travel to locations in Rochester, NY
Facility: The Sands-Constellation Heart Institute
Type: Full-Time (40 hours/week)
Schedule: Monday to Friday | 8:00 AM – 4:30 PM
Pay Range: $17 – $29 per hour

Position Summary

We are currently seeking a dedicated and adaptable Licensed Practical Nurse (LPN) to join our outpatient cardiology team. This full-time role offers consistent daytime hours with no weekend shifts, and the opportunity to float between several clinic locations in Rochester.

Key Responsibilities

• Provide direct nursing support to cardiology patients in a clinical outpatient setting

• Conduct medication prior authorizations and assist with patient care coordination

• Perform in-basket work and documentation using Epic EMR

• Collaborate closely with physicians, nurses, and administrative staff to ensure quality care

• Maintain accurate and timely patient records

• Float between the following locations as needed:

  • Brighton – Clinton Woods (Primary)

  • Portland Ave

  • Linden Oaks

  • Greece Canal Landing

Qualifications

• Active New York State LPN license (required)

• Proficiency in Epic EMR, especially in-basket and documentation tasks

• Previous experience in outpatient care or cardiology preferred

• Strong organizational and communication skills

• Ability to work independently, adapt to multiple locations, and handle feedback constructively
