The Permanency Specialist II delivers permanency services using the Hillside Family Finding Model. This includes collaborating with strategic partners, local districts of social services, community stakeholders, clinicians, care coordinators, etc. to develop an emotional, relational, and legal permanency plan for youth. Permanency Specialist II engages family members and other appropriate caregiving adults to build a lifetime network of support for each youth and family. The Family Finding Permanency Specialist II may be requested to participate in or lead projects, workgroups, and/trainings, and mentor Family Finding Permanency Specialist I staff.

Responsibilities:

  • Increase connections for youth and families enrolled in Family Finding services by creating a network of support for youth and families to assist in sustaining permanency.
  • Educate youth, families, and providers about the positive impact of Family Finding permanency services.
  • Prepare youth, families, and providers for the Family Finding process, which includes gaining permission and buy-in.
  • Participate and provide expertise in Service Plan Reviews, Treatment Conferences, and Benchmark team meetings as it relates to permanency.
  • Utilize experience, education, knowledge of developmental stages and cultural humility to assist children, youth, and families in achieving and sustaining permanency outcomes.
  • Act as the primary contact for the youth and family’s permanency team.
  • Develop, maintain, and update all required documentation according to agency policies and protocols.
  • Adhere to the highest ethical and professional conduct standards at all times.

Requirements:

  • Associate’s degree in a Human Services related field required.
  • 3+ years’ experience in a human services or a related field required
  • Unrestricted, valid NYS driver’s license with a clean driving record and minimum insurance coverage
    that meets agency standards.

CPR certification required to obtain within 60 days of hire

$18.36 Minimum Rate per hour, $27.54 Maximum Rate per hour, based on experience.
